Flu vaccines, both injections and nasal mist, will be available on Tuesday, October 27th from 8 to 10 a.m. Students will need to have a completed consent form (filled out on both sides) to participate. Parents should attach a copy of their insurance card to the consent form. The school will bill insurance.  In addition, the school pays for students who have insurance that is not accepted by the school’s provider.
This clinic is also a Vaccines for Children Program. Children with Medicaid should provide their Medicaid number at the bottom of the consent form. Those who have no insurance may also participate through the Vaccines for Children Program.  Attached are information forms and consent forms in English and in Spanish.  If you are interested in participating, please have the completed consent form turned in to our nurse by this Friday (October 23rd.)  If you have any questions regarding the clinic, please email our school nurse:  Sara.Dahms@grandcenterartsacademy.org
